Managing Personal Information, Medical History, and Health Records

This guide provides a clear step-by-step explanation of how to manage Personal Information, update Medical History, and add or view Health Records, such as Medications, Vaccinations, Allergies, Documents & Imaging, and Injury Reports.

1. Updating Personal Information


  1. Access Personal Information

    • Log into the Olymedic app or website.

    • On the dashboard, find the Personal Information section.


  2. Edit Details

    • Click the label or arrow icon next to the information you want to update:

      • Weight

      • Height

      • Blood Type

    • Enter the new information and save.

2. Editing Medical History


  1. Access Medical History menu

    • On the dashboard, view the Medical History menu.

    • From here, you can directly edit:

      • Medical History

      • Family Medical History

      • General Practitioner


  2. Update Records

    • Select the record you want to edit.

    • Click the label, make the necessary updates, and save your changes.

3. Managing Health Records under Medical History Menu

Within the Medical History menu, you can access variable of health records. Each section includes a list view where you can review existing records or add new records.


3.1 Injury Reports


  1. View Injury Records

    • Access the Injury Reports section to view past injuries reported by you and your doctors.


  2. Add a New Injury Report

    • Click Add New and enter:

      • Injury Description (e.g., "Sprained ankle")

      • Date of Injury

      • Additional details about treatment or recovery.

    • Save the injury report.


3.2 Documents & Imaging


  1. View Existing Documents

    • Open the Documents & Imaging section to view uploaded reports like MRIs, blood tests, or other medical files, reported by you and your doctors.


  2. Add a New Document

    • Click Upload New and fill in the following fields:

      • Document Name (e.g., MRI Report, Blood Test Results)

      • Document Type (e.g., PDF, Image)

      • Additional Information (e.g., date or notes)

    • Choose a file from your device and click Save.


3.3 Medications


  1. View Medications

    • Access the Medications section to review your prescription history, including current medications reported by you and your doctor


  2. Add a New Medication

    • Click Add New and provide:

      • Drug Type (e.g., tablet, capsule)

      • Drug Name (e.g., Ibuprofen)

      • Duration (e.g., 7 days)

      • Dosage Instructions (e.g., "Take 1 tablet after meals.")

    • Save the medication.


3.4 Vaccinations


  1. View Vaccination Records

    • Open the Vaccinations section to view previously recorded vaccinations reported by you and your doctor


  2. Add a New Vaccination

    • Click Add New and enter:

      • Vaccine Name (e.g., COVID-19, Tetanus)

      • Date Administered

      • Additional notes (if applicable).

    • Save the vaccination record.


3.5 Allergies


  1. View Allergies

    • Open the Allergies section to view known allergies.


  2. Add a New Allergy

    • Click Add New and fill in:

      • Allergy Name (e.g., Penicillin, Pollen)

      • Additional Information (e.g., reaction severity or type).

    • Save the allergy.


3.6 General Practitioner


  1. View Practitioner Information

    • Access the General Practitioner section to review or update contact information.


  2. Add or Edit Details

    • Click Add New or select an existing entry to edit.

    • Provide the following:

      • Practitioner Name

      • Email Address

      • Phone Number

    • Save the updated details.
